Job Description
JOB TITLE: Case Manager
Full time; 40-hour work week
Wage: Starting at $46,800 annually
Review date: Open until filled
Reports to: Executive Director
Purpose:
The Case Manager will respond to self-referrals from homeless individuals and to community referrals from hospital emergency rooms, jails, shelters, and other referral sources. This position will provide the bridge to services for individuals who are currently unengaged with any support services. The position will be based largely at the Just Compassion Resource Center and in the community, providing case management for homeless individuals throughout East/South Washington County and will follow them through the steps of engagement and their entry into mainstream services. Develop caseload based on geographical and severity of needs.
Staff is strongly recommended to be vaccinated.
Flexible schedule is a must to fit the needs of individuals utilizing our programs.
Accountabilities:
Outreach
  • Participate in regular case management activities to connect with disenfranchised population.
  • Identify individuals in that population who meet the definition of homelessness.
  • Conduct pre-engagement and engagement meetings as needed, coordinate intake and assessment for mental health, addiction services and other services as appropriate.
  • Identify barriers to service and support, such as transportation problems, or need for emergency assistance (clothing, blankets, etc.), design short-term solutions for meeting individual's basic needs.
  • Design tentative individual service and support plan identifying individual's most urgent needs and long-term goals.
  • Respond to referrals from community partners and stakeholders within the
Tigard, Sherwood area. Engage guests to Just Compassion and services as appropriate.
Case Management
  • Develop case plan with goals and objectives with timelines
  • Assist individual to gain access to mental health/physical health/addiction services by linking with an appropriate health agency.
  • Assess for housing needs and help link individuals as needed with shelter, transitional, or permanent housing.
  • Assess for eligibility for public entitlements and assist individual to apply and/or refer individual to ASSIST.
  • Follow individual through the first steps of engagement and referral to permanent caseworkers/service and support resources and ensure coordinated transition before terminating with individuals.
  • Identify individuals who "fall through the gaps" because of funding problems; refer to Just Compassion for additional support.
  • Meet regularly with other Just Compassion team members to ensure that a wrap-around approach for case management is being incorporated
Administrative
  • Enroll all individuals in HMIS and other appropriate systems
  • Document all activities provided in HMIS and other appropriate systems
  • Provide quarterly and yearly reports as required by funding agencies
  • Attend all agency and community meetings as assigned; collaborate with other providers to ensure seamless services to this population
  • Provide training and information referral on homeless and mental health, addiction, housing resources to community providers
  • Other reporting or tasks as delegated by the Executive Director
Misc.
  • Support volunteer coordination and HMIS data entry for annual PIT Count
  • Work collaboratively with other coordinators and staff at Just Compassion
  • Order and budget supplies for outreach activities, with approval from
Executive Director
  • Strength in advocacy work for at-risk populations. Ability to advocate for guests and use power/privilege to advocate for the resources they need.
  • Ability to think creatively to find solutions to reduce barriers.
  • During Shelter season working during shelter hours to help meet the needs of individuals staying in our shelter.
Other General Expectations:
  • Attend and positively participate in required meetings.
  • Pursue ongoing development of skills and competencies.
  • Follow all corporate policies, procedures, and protocols.
  • Maintain current Oregon Driver’s License and insurance.
  • Follow through on all directives and instructions.
  • Maintain security of all corporate interests.
Positions Reporting: None
Education/Certificates/Licenses/Registration:
High School Dimploma or equivalent required. QHMA credentials a plus, Bachelor of Arts or Science in Psychology or a related field preferred. Experience working with adults diagnosed with mental illness, drug dependencies and homeless individuals. Computer literacy is required. Valid driver’s license and insurance are required.
Experience/Qualifications:
Must have experience providing case management to homeless individuals or other vulnerable populations and be familiar with the needs of homeless mentally ill and drug affected individuals. Strong motivational interviewing skills are needed.
Must possess the ability to interface with other agencies and represent Just
Compassion in a professional manner. Must be able to work independently. Ability to develop and follow policies and procedures for field work in an unstructured setting desired.
